The Community Transition Support (CTS) initiative aims to improve post school outcomes for young people with a disability who attend a specialist school into further education, training, employment and community participation.
CTS is funded by the North and West Division of the Department of Health and Human Services and works across the North and West Metropolitan region of Melbourne.
The initiative aims to build the capacity of the 22 specialist schools across the region to improve career development practice and broker local strategic partnerships to enhance opportunities for the whole of school community.
The CTS team is working with a range of services across the region including:
- Fourteen Local Government Authorities (LGA’s)
- Further Education and Training Providers such as TAFE, Registered Training Organisations, Group Training Organisations
- Community Service Providers including Disability Employment Services, Not For Profit community services, Neighbourhood Houses and
- Industry and Business
